Please follow the steps below to schedule or check-in for your appointment with Carencia. Before booking online, please take a moment to review our Appointment Agreement policy.
Accessing OnPatient Portal
- Login to OnPatient portal using the link below
- Enter your Email and Password or select Sign Up to create a new account
- Once logged in, select the Appointments tab at the top
- Find the upcoming appointment in the list that you’d like to check-in for
- Select Check In from the far right
- Review and Update any information presented on the Onboarding Forms
- Select I’m Done button at the bottom to finish the check-in process
